红细胞在体内的衰亡和破坏的机制仍未完全清楚。正常人体内的红细胞寿命平均为120天，主要是因衰老而消失。另有极少数红细胞可被其他因素导致红细胞的变形性下降或细胞表面性质改变而过早破坏。成熟的红细胞在长期存活过程中逐渐衰老，表现在细胞膜的蛋白质脂质含量、红细胞酶活性、糖酵解能力下降、物质交换及能量转换均逐渐减少，对红细胞的重要生理功能有不良影响。老龄红细胞的唾液酸含量也减低，更可影响红细胞的寿命，再加上膜表面电荷密度减少，表面积和容积的比值下降，细胞呈球形易被巨噬细胞识别吞噬。
