大脑半球（cerebral hepIMC），端脑的两侧结构。两半球前后完全分开，中间由一束最大的连合纤维——胼胝体连接。表面是灰质，即大脑皮层，其隆起的部分为脑回,凹陷的部分称沟裂。背外侧面是由大脑外侧裂和中央沟等分成的额、顶、枕、颞等脑叶。最前端是额极，最后端是枕极。皮质下是厚的白质，即大脑髓质。据构成的路径及其联系，髓质纤维束可分为三类：投射纤维、半球内的联合纤维和半球间的连合纤维。投射纤维是皮质与皮质下中枢的联络纤维，包括传导冲动到皮质的上行纤维束以及将冲动从皮质传到皮质下中枢的下行纤维束。上行纤维主要是丘脑放射、昕放射和视放射，下行纤维主要为锥体系和锥体外系。
