推荐者应以书面形（中英文均可，以500字为限），将所推荐的教师在2005内的模范事迹填写在表格上，并将它于2006年1月31日前送达或寄交主办当局。
Each nomination has to be supported by an English or Chinese write-up of maximum 500 words on the nominee's achievements in 2005. The submission has to reach Lianhe Zaobao by 31 January 2006.
