谷丙转氨酶（ALT）主要存在于各种细胞中，尤以肝细胞为最，整个肝脏内转氨酶含量约为血中含量的100倍。正常时，只要少量释放入血中，血清中其酶的活性即可明显升高。在各种病毒性肝炎的急性期、药物中毒性肝细胞坏死时，ALT大量释放入血中，因此它是诊断病毒性肝炎、中毒性肝炎的重要指标。肝细胞内谷丙转氨酶的浓度比血清高1000～3000倍。只要有1%的肝细胞坏死，便可使血中酶活性增高1倍，因此转氨酶（尤其是ALT）是急性肝细胞损害的敏感标志。
