规则散光是由于角膜或晶体的两个主要经线的弯曲度（即屈光力）不同所造成。这两个主要经线互相垂直，其中一个弯曲度最大，屈光力最强；另一个弯曲度最小、屈光力最弱，其他经线的屈光力则自最大屈光力经线向最小屈光力经线顺序递减。因此，平行光线通过规则散光的屈光系统屈折后，不能形成焦点而是在两个互相垂直的经线上形成前后两条焦线。
