腰椎结核俗称“龟背炎”，发病率较高，为全身骨关节结核的第一位，约占骨关节结核总数的一半，其中以儿童和青少年发生最多。所有脊柱均可受累，以往以腰椎结核最多，近年来以胸椎多见，腰椎次之，其次是骶椎和颈椎等。椎体结核占绝大多数，单纯附件结核少见。腰椎结核脓肿常至盆腔，形成腰肌脓肿，沿髂腰肌向下蔓延到腹股沟或股内侧，从股骨后达大粗隆，沿阔筋膜张肌和髂胫束至股外侧下部，或向后蔓延到腰三角区，形成所谓寒性脓肿。椎体病变因循环障碍及结核感染，有骨质破坏及坏死，有干酪样改变和脓肿形成，椎体因病变和承重而发生塌陷，使脊柱弯曲腰背部可出现“驼峰”畸形。由于椎体塌陷，死骨肉芽组织和脓肿形成，可使脊髓受压或血供受累而发生截瘫。
