是指各种形式的椎管、神经管以及椎间孔的狭窄，及软组织引起的椎管容积改变及硬膜囊本身的狭窄等引起的一系列腰腿痛及一系列神经系统症状出现。称为腰椎管狭窄症。因为椎管的狭小，压迫了位于椎管中的马尾神经产生腰腿痛等症状。如果椎管侧方狭窄则神经根也会受到嵌压，引起轴浆因受压中断；神经体液运转障碍；神经鞘膜相对膨胀；刺激神经末梢；又因血运受阻则组织缺氧；静脉回流受限局部淤滞等，这些构成产生腰腿疼的病因。腰椎管狭窄症是指因原发或继发因素造成椎管结构异常，椎管腔内变窄，出现以间歇性跛行为主要特征的腰腿痛。
