脾脏是一个血供丰富而质脆的实质性器官，被与其包膜相连的诸韧带固定在左上腹的后方，有下胸壁、腹壁和膈肌的保护。外伤暴力很容易使其破裂引起内出血。脾是腹部内脏中最容易受损伤的器官，发生率几乎占各种腹部损伤的20%～40%，已有病理改变（门脉高压症、血吸虫病、疟疾、淋巴瘤等）的脾脏更容易损伤破裂。脾破裂分为外伤性破裂和自发性破裂。脾损伤分级：Ⅰ级 脾被膜下破裂或被膜及实质轻度损伤，术中见脾裂伤长度≤5.0，深度≤1.0厘米；Ⅱ级 脾裂伤长度>5.0，深度>1.0厘米，但脾门未累及，或脾段血管受累；Ⅲ级 脾破裂伤及脾门部或脾部分离断，或脾叶血管受累；Ⅳ级 脾广泛破裂，或脾蒂、脾动静脉主干受累。
