结核杆菌经血行感染进入肾，主要在双侧肾皮质的肾小球毛细血管丛内，形成多发性微小结核病灶。由于该处血循环丰富，修复力较强，如病人免疫状况良好，感染细菌数量少或毒力较小，这种早期微小结核病灶可以全部自行愈合，临床上常不出现症状，称为“病理肾结核”。但此期可在尿液中查见结核杆菌。
