截骨术是指 将坏死区移出股骨头负重区，将未坏死区移出负重区。应用于临床的截骨术包括内翻或外翻截骨、经股骨转子旋转截骨术等。该方法适用于坏死体积中等的II期或III期早、中期的AVN。此术式会为以后进行人工关节置换术带来较大技术难度。
