慢性结石性胆囊炎在不同部位的胆石，其形态、大小和成分等一般都有明显的差别，因此其形成的机制也是不同的，而其临床症状和病理表现也将因人而异。60%的病人无明显临床表现，于查体或行上腹部其他手术而被发现。当结石嵌顿引起胆囊管梗阻时，常表现为右上腹胀闷不适，类似慢性胃炎症状，但服用治疗胃炎药物无效，病人多厌油腻食物；当病人于夜间卧床变换体位时，结石堵塞于胆囊管处暂时梗阻而发生右上腹和上腹疼痛，因此常有夜间腹痛。
