张力性气胸是指较大的肺气泡破裂或较大较深的肺裂伤或支气管破裂，裂口与胸膜腔相通，且形成单向活瓣，又称高压性气胸。吸气时空气从裂口进入胸膜腔内，而呼气时活瓣关闭，腔内空气不能排出，致胸膜腔内压力不断升高，压迫肺使之逐渐萎陷，并将纵隔推向健侧，挤压健侧肺，产生呼吸和循环功能的严重障碍。胸膜腔内的高压空气若被挤入纵隔，扩散至皮下组织，形成颈部、面部、胸部等处皮下气肿。
