巩膜移植术（scleroplasty）或后巩膜支撑术（posteriorscleralsupport）矫治高度近视，应用一些保护材料支撑眼球的后极部，以防止后极部组织的进一步扩张，保护已被损伤的视网膜，该手术只适用一部分伴有黄斑变性及因巩膜扩张致高度近视进展过快的高度近视患者，及遗传性恶性近视眼。术后并发症有视网膜出血、葡萄膜炎、青光眼等。
