小肠在腹腔内占据的位置最大、分布面广、相对表浅、缺少骨骼的保护容易受到损伤。在开放性损伤中小肠损伤率占25%~30%,闭合性损伤中占15%~20%。患者可表现为剧烈的腹痛,伴有恶心、呕吐。查体可见患者面色苍白、皮肤湿冷、脉搏微弱、呼吸急促、血压下降。可有全腹压痛、反跳痛、腹肌紧张等。
