本品是叶酸还原型的甲酰化衍生物，系叶酸在体内的活化形式。叶酸在小肠细胞内经二氢叶酸还原酶还原并甲基化，转变为甲基四氢叶酸，然后作为辅酶参与体内嘌呤和嘧啶核苷酸的合成及某些氨基酸的转化。甲氨喋呤可与二氢叶酸还原酶结合，阻断二氢叶酸转变为四氢叶酸，从而抑制胸腺嘧啶核苷酸、DNA、RNA以至蛋白质的合成。
