顾名思义，双相兼有心境变高和变低两极性特点，是心境在正常，高涨（躁狂），低落（抑郁）之间往返摆动。虽有单相躁狂（常归为双相之列），但一般所谓单相一概指的是单相抑郁（重性抑郁），是心境往返于正常和抑郁之间。双相障碍又以临床症状的多样性（躁狂症状，抑郁症状及精神病性症状）和病程的复杂多变性著称，极易和精神分裂症，重性抑郁混淆，造成漏诊和误诊，治疗难度也极大。
