下颌骨分为体部及升支部，两侧体部在正中联合。下颌升支部上方有两个骨性突起，在后方者称为髁状突，在前方者称为喙突（肌突），两者之间的凹缘称为乙状切迹。升支部后缘与下颌骨下缘相交处称为下颌角，升支部内侧面中部有一个孔称下颌孔，此孔在下颌骨内向下向前延伸的管道，称下颌管。 下颌管在第一、第二前磨牙牙根之间向外穿出一孔，称颏孔。下牙槽神经、血管从下颌孔进入下颌管向前走行，在颏孔处分出颏神经及血管。正常的下颌角的角度是120°，外观看起来应没有棱角，而下颌角肥大者，包括两方面的情况：一是下颌角的角度过小，呈现“国”字脸，这在女性就显得不温柔。二是下颌骨外翻增厚，使得脸型呈梯形，也不符合美学的标准，这种情况多伴有咬肌肥厚，病人面形较宽大多呈梯形面形，侧面看呈男性的下颌角，缺乏柔和、流畅的轮廓。既往治疗下颌角采用口外切口，遗留疤痕，而且截骨线不平整，外观线条不流畅，特别是小数情况下可能损伤面神经。多采用口内磨骨的方法，可以避免并发症的发生。治疗下颌角肥大，采用局部麻醉，通过口内 3cm 切口，对下颌骨进行磨除，由于视野清楚出血很少，不会损伤面神经，磨骨面非常流畅。手术的同时可以去颊脂垫和隆下颌。
