三房心系胚胎期原发隔异常发育，形成左心房内有畸形隔膜，使左心房分为副房及真正左心房两个部分。亦可能系肺静脉总干与左心房融合不良，由肺静脉总干形成副房。由于上述原因，有人认为三房心亦可视为一种完全性肺静脉异位与左心房连接异常的病变。
