In a patient with confirmed vasculitis, the presence of three of more of the following criteria are sensitive and specific for the diagnosis of giant-cell arteritis: an age over 50 years at onset, new headaches, a temporal artery abnormality on physical examination (tenderness on palpation or decreased pulsation), an elevated erythrocyte sedimentation rate (50 mm per hour or higher), and an abnormal finding in a specimen from a temporal artery biopsy. In a patient with suspected giant-cell arteritis and impending visual loss, immediate corticosteroid therapy is indicated. Temporal artery biopsies should occur within a week.
