胆脂瘤型中耳炎在2012年中华医学会的《中耳炎临床分类和手术分型指南》中更改为中耳胆脂瘤。以前曾经认为胆脂瘤型中耳炎是慢性化脓性中耳炎中最重的类型，但实际上本病为鳞状上皮组织在中耳、乳突内的生长，其生成机制、病理及转归与慢性化脓性中耳炎不同。所谓胆脂瘤，就是脱落上皮的堆集，越聚越大，不断地向四周扩张，从而造成邻近的骨质破坏，有可能造成周围器官的并发症。因此本病的治疗是一经诊断应建议手术，且手术的主要目的不是为了提出高听力及不流脓，而是预防并发症。
