白内障囊外摘除术是指在同轴光照明下的显微手术，其手术方式是刺破并撕去前囊中央部分，将晶体核娩出，用白内障同步注吸针头吸净周边囊袋内的皮质，保留完整的晶体后囊和周边的前囊。白内障囊外摘除术的主要优点是保留了晶体后囊，便于植入和固定人工晶体，适合于成年人核性白内障。目前在发达地区，囊外摘除术和人工晶体植入术是主要的手术方式。
