For enrolment, please submit one Registration form for each place in each course. This form may be photocopied for use.
每位报名者每一项课程请递交一份报名表格，表格可影印备用。
Course fee must be accompanied with this form (or photocopy) before course commence, otherwise enrolment would be rejected.
必须连同费用于开始前一并缴交，否则报名无效。
