系原发性或继发性关节软骨退行性改变。关节软骨磨损，关节间隙变窄，软骨下骨质硬化，关节边缘骨赘形成。原发性病变发生于50岁以后，女略多于男，多关节受累，以负重关节为主。继发性病变发病于40岁左右，单关节受累。关节镜检查可见关节滑膜绒毛增生和炎症，关节软骨粗糙、溃烂、纤维化甚至缺失。本病贵在预防，适当体育活动，防止关节长时间超负荷使用，及时矫正畸形或其他致病因素。起病后注意保暖，可物理治疗。试用关节镜下关节腔灌洗术或关节清理术，减少有害物质对关节的刺激和损害，以缓解症状和减慢病情进展。疼痛持续或畸形严重影响工作甚或生活者，可考虑行截骨术、关节成形术、关节融合术或人工全关节置换术。
