《辞源》解释“面首”为：“面，貌之美；首，发之美。面首，谓美男子。引申为男妾、男宠。”确定面首这个称谓的，是南北朝时期南朝刘宋的前废帝刘子业。根据《宋书·前废帝纪》记载：山阴公主淫恣过度，谓帝曰：“妾与陛下，虽男女有殊，俱托体先帝。陛下六宫万数，而妾唯驸马一人。事不均平，一何至此！” 而“帝乃为主置面首左右三十人。”
