脑萎缩（brain atrophy）是指由各种原因导致脑组织本身发生器质性病变而产生萎缩的一种现象。病理上表现为脑组织体积缩小，细胞数目减少，脑室和蛛网膜下腔扩大。本病多发生于50岁以上，病程可达数年至数十年，男性多于女性，可分为弥漫性脑萎缩（包括皮层萎缩、小脑萎缩及皮层、小脑、脑干萎缩）及局限性脑萎缩（多见于局限性脑器质性病变后如外伤、血管病、颅内局限性感染等）。脑萎缩原本是在CT或MRI检查时，发现脑组织体积减小和脑室扩大等而命名的，因此，脑萎缩是许多疾病的神经影像学表现。
