脊柱结核在全身骨关节结核中发病率最高，约占骨关节结核总数的一半，其中以儿童和青少年发生最多。所有脊柱均可受累，以往以腰椎结核最多，近年来以胸椎多见，腰椎次之，其次是骶椎和颈椎等。椎体结核占绝大多数，单纯附件结核少见。胸椎结核常形成椎前和椎旁脓肿，也可出现在后纵隔区或沿肋间向胸壁发展。椎体病变因循环障碍及结核感染，有骨质破坏及坏死，有干酪样改变和脓肿形成，椎体因病变和承重而发生塌陷，使脊柱弯曲，腰背部可出现“驼峰”畸形。由于椎体塌陷，死骨肉芽组织和脓肿形成，可使脊髓受压或血供受累而发生截瘫。
