胃大部切除术也称“胃次全切除术”。包括切除幽门窦全部在内的胃组织的3/5～4/5，主要用于治疗胃十二指肠溃疡及胃肿瘤。多采用硬膜外麻醉，取上腹正中切口，手术步骤：①游离胃大、小弯及切断十二指肠;②切胃;③胃肠道重建：根据胃肠道重建的方式可分为毕I式(即胃-十二指肠吻合)及毕Ⅱ式(即胃-空肠吻合)。术后并发症发生率为5%～15%，包括胃出血、十二指肠残端破裂、输出、入段梗阻、倾倒综合征、反流性胃炎、吻合口溃疡、营养性并发症及残胃癌等。本手术疗效好，优良率达85%～90%。
