直接胆红素（DBil）又称结合胆红素，是由间接胆红素进入肝后受肝内葡萄糖醛酸基转移酶的作用与葡萄糖醛酸结合生成的。直接胆红素溶于水，与偶氮试剂呈直接反应，能通过肾随尿排出体外。肝脏对胆红素的代谢起着重要作用，包括肝细胞对血液中间接胆红素的摄取、结合和排泄三个过程。血清直接胆红素的升高，说明经肝细胞处理后胆红素从胆道的排泄发生障碍。直接胆红素的测定有助于黄疸类型的诊断和鉴别诊断。
