受害者处于不可逆的深昏迷状态,丧失意识活动,但皮质下中枢可维持自主呼吸运动和心跳,此种状态称“植物状态”. 植物状态是由各种病因引起的严重脑损伤后有觉醒但无觉知的状态。1972年之前,对这类患者无统一的疾病命名,最常用的病名为去皮质综合征(apallic syndrome),也有无动性缄默症(akinetic mutism),创伤后痴呆(post-traumatic dementia)或睁眼昏迷(coma vigil)等叫法。1972年,Jennett和Plum使用植物状态(vegetative state)一词命名这类患者。“植物”的意思是,这类患者有基本的生理功能如呼吸、心跳、血压、体温维持、消化功能等,但对自我和外界环境没有觉知。持续性植物状态(persistant vegetative state)指的是植物状态持物1月以上,意识并非没有恢复可能。永久性植物状态(permanent vegetative state)指外伤性植物状态持续1年或非外伤性植物状态持续3月以上,意识恢复可能性不大。然而数十年后恢复意识的植物状态患者的案例也时有报道。2010年,无反应觉醒综合征(unresponsive wakefulness syndrome)被推荐取代植物状态一词,以避免“植物”给人的消极印象。这类患者的发病机制、诊断以及治疗仍然困难重重,需要更多、更深入的临床和基础研究。
