小脑萎缩（cerebellar atrophy）准确来说不是一种疾病，而是一种神经影像学的表现。既可见于一些遗传性、变性性疾病，也可见于某些急性病程如急性小脑炎的后期及某些药物中毒等，甚至某些临床无症状的人，影像学检查也可见到小脑萎缩，尤以老年人多见。其共同特征是神经影像学检查发现小脑的容积减小，脑沟增宽。可分为局限性和广泛性小脑萎缩。由于小脑主要参与躯体平衡和肌肉张力的调节，因此小脑萎缩患者临床多出现步态不稳，共济失调，言语不清等症状。
