此病可能发生于青春期以后的任何年龄。病变是以腺体小叶和乳腺导管末梢扩张、增生和囊性改变为主的病理过程。它的发生和发展与卵巢内分泌功能紊乱有关，多由于孕激素分泌不足和雌激素相对或绝对过多，致使月经周期中乳腺组织的增生和复旧过程发生紊乱而罹病。而不良精神刺激导致的郁郁寡欢、孤独焦虑则是乳腺小叶增生的“催化剂”，多见于25～45岁月经不太规则、经量少、经期短、未婚、未育、或未哺乳和哺乳不良的妇女。
可见到囊性扩张导管、小叶区伴大量纤维结缔组织（硬化性腺病）增生、间质纤维化。
