胃的癌前病变是指一类容易发生癌变的胃黏膜病理组织学变化，即胃黏膜的异型增生和肠上皮化生，主要伴存于慢性萎缩性胃炎。 胃癌是我国最常见的恶性肿瘤之一，严重危害人类健康。而胃黏膜癌肿不是由正常细胞“一跃”变成癌细胞，而是一个多步骤癌变的过程，即慢性浅表性胃炎→萎缩性胃炎→肠上皮化生→异型增生→胃癌，在这期间出现的病变称之为癌前病变。因此临床上常把伴肠上皮化生、异型增生称之为慢性萎缩性胃炎癌前病变或胃癌前期病变。伴中度以上的异型增生和不完全大肠型化生则称之为真正的胃癌癌前病变。 慢性萎缩性胃炎容易发生癌变，那么发生癌变的概率是多少呢﹖一般认为其癌变率是：5～10年癌变率为3%～5%，10年以上为10%，轻度异型增生10年癌变率为2.5%～11%，中度异型增生10年癌变率4%～35%，重度异型增生10年癌变率为10%～83%。
