肿瘤组织在细胞形态和组织结构上，都与其来源的正常组织有不同程度的差异，这种差异称为异型性(atypia)。异型性的大小可用肿瘤组织分化成熟的程度来表示。分化(differentiation)在胚胎学中指原始幼稚细胞在胚胎发育过程中，向不同方向演化趋于成熟的程度。病理学将此术语引用过来，指肿瘤细胞与其发生部位成熟细胞的相似程度。肿瘤细胞异型性小，表示它和正常来源组织相似，分化程度高，则恶性程度低。反之，肿瘤细胞异型性大，和正常来源组织相似性小，肿瘤细胞分化程度低，往往其恶性程度高。异型性是判断良、恶性肿瘤的重要组织学依据。间变(anaplasia)在现代病理学中指肿瘤细胞缺乏分化的状态。由未分化细胞构成的恶性肿瘤称间变性肿瘤。间变性肿瘤多为高度恶性的肿瘤。
