皮质脊髓束（corticospinal system），是人类脊髓中最大的下行神经纤维束。其神经元在大脑皮质的中央前回运动区，及皮质的其他区域，其纤维在同侧内囊后脚汇集成束，下行至延髓形成锥体，在锥体下端大部分交叉至对侧，形成皮质脊髓侧束，在脊髓外侧索下行，并直接或间接止于前角运动神经元；小部分在锥体未交叉的纤维则形成皮质脊髓前束，在脊髓前索下行，同样直接或间接终止于前角细胞。此束的功能是支配骨骼肌的随意运动，它与前角运动神经元一起组成随意运动的传导通路。
