病变位于螺旋器的毛细胞、听神经或各级听中枢,对声音的感受与神经冲动的传导发生障碍,所引起的听力下降即为感音神经性耳聋。其中毛细胞病变引起者称感音性聋(耳蜗性聋或终器性聋)常有重振现象,病变位于听神经及其传导径路者称神经性(蜗后性聋或之后性聋),其特点为语言辨别率明显下降。病变发生于大脑皮层听中枢者称中枢性聋,常伴有其经神经系统症状。
