佛教中有六道轮回之说，奈何桥在中国佛教观念中是鬼魂历经十殿阎罗的旅途后准备投胎的必经之地，在这里会有一名称作孟婆的年长女性神祇，给予每个鬼魂一碗孟婆汤以遗忘前世记忆，好投胎到下一世。奈何梵文，意即地狱。传说死者到此，有罪的要被两旁的牛头马面推入“血河池”遭受虫蚁毒蛇的折磨，而行善之死者过桥，却非常简单。而“奈何”两个字，在中文里，也正好有无可奈何、毫无办法的意思。有诗曰：生死黄泉路迢迢，孤影徘徊魂渐消 。浮望三生石前事，今日又过奈何桥。
