急性红血病表现以原红细胞、早幼红细胞的恶性增生为主，可见类巨幼变。临床上常误诊为巨幼细胞贫血。急性红白血病则表现为红、白(主要是粒)两系的恶性增生，最后可发展成为典型的急性粒细胞白血病或急性粒、单核细胞白血病。一般认为红血病可以发展成红白血病，后者可再进一步转化为急性白血病，但临床上不是每个病例都有这样的转化过程，有的病例可能在未转化前就死于并发症，因而未能显示出病情发展的全过程。
